Case Manager  - Lisa Monks

Lisa is in charge of the EISEP program that provides housekeeping and personal care services to senior citizens. She conducts assessments in order to evaluate the needs of each individual and makes referrals to local agencies for care. Lisa maintains on-going communication with clients, agencies, and families to ensure proper care is being given.
